While electronic cigarettes (e-cigarettes) are often considered less harmful than conventional tobacco cigarettes, they are not risk-free and may pose a cancer risk. The aerosol produced by e-cigarettes is not simply water vapor; it contains several potentially harmful substances, including known carcinogens.

Harmful Components in E-cigarette Aerosol

Research has identified various chemicals in e-cigarette aerosol that are linked to cancer, although often at lower levels than in traditional cigarette smoke. These include:

  • Formaldehyde and Acetaldehyde: These are known carcinogens that can form when the e-liquid solvents (propylene glycol and vegetable glycerin) are heated to high temperatures.
  • Acrolein: A highly irritating substance that can damage DNA and is a known carcinogen.
  • Volatile Organic Compounds (VOCs): Some VOCs found in e-cigarette vapor, such as benzene (a known carcinogen), can contribute to cancer risk.
  • Heavy Metals: Particles of metals like nickel, tin, lead, and chromium, which can be carcinogenic, may leach from the heating coil or other device components into the aerosol.
  • Nitrosamines: Some tobacco-specific nitrosamines (TSNAs), potent carcinogens, have been found in certain e-liquids and their aerosols, particularly those containing nicotine extracted from tobacco.

Understanding the Risk

The overall cancer risk from e-cigarettes is still being actively investigated. Most experts agree that e-cigarettes likely pose a lower cancer risk than smoking traditional cigarettes because they expose users to fewer carcinogens and at lower concentrations. However, “lower risk” does not equate to “no risk.”

Long-term studies are crucial to fully understand the cancer risks associated with prolonged e-cigarette use, as cancers often take many years to develop. Current evidence from laboratory studies on cells and animals suggests that e-cigarette aerosol can cause DNA damage and promote cellular changes that could lead to cancer. The specific risk can also vary depending on factors such as the type of device, the e-liquid ingredients, voltage/power settings, and user behavior (e.g., puffing intensity and frequency).

Therefore, while switching from combustible cigarettes to e-cigarettes might reduce exposure to many carcinogens, the safest approach is to avoid using any tobacco or nicotine products, including e-cigarettes, especially for non-smokers and young people.
