Public Health England maintains that e-cigarettes are significantly less harmful than traditional tobacco cigarettes. However, the NHS clearly states they are not risk-free and are not recommended for non-smokers and young people.
Key Health Concerns
- Nicotine Addiction: Most e-cigarettes contain addictive nicotine, harmful to adolescent brain development and sustaining dependence.
- Unknown Long-Term Effects: As a relatively new product, the long-term health impact requires further ongoing research.
- Potential Toxicants: Vapour can contain potentially harmful substances like formaldehyde, acetaldehyde, and acrolein, albeit generally at much lower levels than cigarette smoke. Heavy metals may also be present in trace amounts.
- Short-Term Side Effects: Users commonly report throat/mouth irritation, dry cough, headaches, dizziness, and nausea.
- Lung Irritation & Injury: Ingredients (including flavourings and solvents) can irritate airways. Rare cases of severe lung injury (e.g., EVALI) have been linked, primarily to illicit THC oils containing Vitamin E Acetate.
- Dual Use Risks: Using both e-cigarettes and tobacco cigarettes concurrently increases health risks compared to stopping smoking completely.
- Child & Youth Risks: Nicotine poisoning risk from ingestion, potential gateway to smoking, adverse effects on developing brains.
NHS Position on Smoking Cessation
The NHS supports licensed nicotine replacement therapy (NRT) like patches and gum as the first-line treatment for quitting smoking. E-cigarettes are not currently available on prescription. However, the NHS advises that for adult smokers unable to quit with other methods, switching completely to e-cigarettes is substantially less harmful than continuing to smoke. Vaping combined with behavioural support offers the best chance of quitting tobacco successfully.

Important Considerations
- Not Risk-Free: “Less harmful” does not mean harmless. The best option for health is avoiding both tobacco and e-cigarettes.
- Non-Smokers: Should not start vaping due to potential health risks and addiction.
- Regulation: Ensure purchasing from reputable sources meeting UK safety standards.
